All your paperwork can be conveniently filled out on one of our tablets so it is immediately available and on file, or you can fill out your New Patient forms via the secure link below. You’ll meet the team and, of course, receive your comprehensive new patient exam. You will also meet with the hygienist and Dr. Brown to discuss your current oral health, what you are hoping to achieve, and the findings from your exam. This is one of the most important visits of your time with Smiles by Design, so be sure you let us know what you want and need. We want to know you as a person, not just a set of teeth!

Everyone in your family will appreciate the way we strive to keep you comfortable and all the little extras we provide to help you relax. Our reception area is cozy and beautifully designed, so it never feels cold or clinical. Soothing music is always played throughout the office to set the tone for relaxation. If you feel a little thirsty, we have coffee, tea, and a water cooler to refresh you and keep you going. Once you are in the chair, ask for one of our snuggly blankets and a neck pillow. You’d be surprised at how comforting something as simple as a throw over your lap can be! And when your appointment is over, freshen up with one of our scented towels. You’ll leave feeling great!
